Pharmacy Medication History Technician

POSITION
Summary:
The Pharmacy Medication History Technician is responsible for accurately collecting and entering current patient medications into the hospital information system.
# This is utilized by the physicians and providers responsible for admitting and discharging the patient from the hospital.
# EDUCATION/CERTIFICATION:
High School diploma or GED equivalent.
Must have a CT Pharmacy Technician License or be license eligible and have completed and be Pharmacy Technician Certification Board Certified, or, must complete certification within 12 months of employment, Or; If enrolled in a university pharmacy program, must have a CT Pharmacy Intern Registration or complete registration within 12 months of employment.

Experience:
Previous Pharmacy Technician experience is desirable but not required.
Must be a Pharmacy Tech II or working toward that level.
Prior Medication History Technician Preferred.
Retail Experience Preferred.

Must have computer skills and keyboard familiarity for work in the Meditech system.

# Effectively communicates with patients, families, and/or caregivers to perform a patient medication history and construct a pre-admission medication list.
Communication includes patient interviews and review of internal and external systems with relevant information.
Relays any outstanding transition of care issues to the pharmacist and/or physician.
Reconciles and documents the most accurate medication history.
Documents accurately and completely in the medical record.
Consults with Pharmacist for any questions before documentation.
Contacts the patient#s outside pharmacy or physician to resolve all discrepancies in a timely manner.
Handles patients# own medications to ensure they are secured and documented.
Performs other duties as assigned in the pharmacy for times of minimal activity.
Utilizes time in most effective and productive manner and completes tasks at the prescribed time.
Completes required continuous training and education, including department specific requirements.
Perform assigned responsibilities according to department policies, procedures, standards and regulations within stated time frames.
May be asked to perform Pharmacy Technician duties to meet department needs.
